Risk-Adjusted Control Charts for Health Care Monitoring
نویسنده
چکیده
Attribute data from high quality processes can be monitored effectively by deciding on whether or not to stop at each time where r ≥ 1 failures have occurred. The smaller the degree of change in failure rate during Out-of-Control one wants to be optimally protected against, the larger r should be. Under homogeneity, the distribution involved is negative binomial. However, in health care monitoring, (groups of) patients will often belong to different risk categories. In the present paper we will show how information about category membership can be used to adjust the basic negative binomial charts to the actual risk incurred. Attention is also devoted to comparing such conditional charts to their unconditional counterparts. The latter do take possible heterogeneity into account, but refrain from risk adjustment. Note that in the risk adjusted case several parameters are involved, which will typically all be unknown. Hence the potentially considerable estimation effects of the new charts will be investigated as well.

Risk-adjusted control charts based on LR-fuzzy data
Control charts are widely used in industrial processes as well as in health sciences and particularly for monitoring the performance of cardiac surgeon or a group of surgeons based on the preoperative risk of patients. Since the preoperative risk is a vague and nonprecise variable and the anesthesiologists after checking how many risk factors a patient has, determine the risk of mortality befor...

Usually, in monitoring schemes the nominal value of the process parameter is assumed known. However, this assumption is violated owing to costly sampling and lack of data particularly in healthcare systems. On the other hand, applying a fixed control limit for the risk-adjusted Bernoulli chart causes to a variable in-control average run length performance for patient populations with dissimilar...

Use of risk adjusted control charts for monitoring patients’ surgical outcomes is now popular. These charts are developed based on considering the patient’s pre-operation risks. Change point detection is a crucial problem in statistical process control (SPC).It helpsthe managers toanalyzeroot causes of out-of-control conditions more effectively. Since the control chart signals do not necessarily...
متن کاملUsing control charts to monitor quality of hospital care with administrative data.
OBJECTIVE Nearly all hospital-specific comparative analyses, based on administrative data, are presented using cross-sectional displays. In this paper, we compare cross-sectional analyses with sequential monitoring using control charts. DESIGN ANALYSIS of administrative data to compare cross-sectional funnel plots with one type of control chart: the risk-adjusted, expected-minus-observed p...

INTRODUCTION A health care system is a complex adaptive system. The effect of a single intervention, incorporated into a complex clinical environment, may be different from that expected. A national database such as the Intensive Care National Audit & Research Centre (ICNARC) Case Mix Programme in the UK represents a centralised monitoring, surveillance and reporting system for retrospective qu...
